    Your business, described accurately

    We ask for business registration, a named contact and your declared credentials. Our verification requirements are being finalised. Applying does not mean your business is approved, listed or entitled to receive cases.

    An introduction to your organisation

    The patient agrees to share their case with one named organisation. That agreement does not give permission to pass it to every hospital in your network. Keep any further sharing specific and separately authorised.

    Before you apply

    Is this a hospital application?

    No. You apply as a medical facilitator, using a provider account. Your business coordinates care; the treating hospital is a separate organisation.

    Will creating an account give me patient details?

    No. Provider approval, a reviewed published profile, matching and the patient’s choice are separate steps. We do not promise a number of enquiries.

    Do I need to choose hospital accreditation?

    No. The facilitator form asks about optional MVTF empanelment status instead. Do not use a partner hospital’s accreditation as your business’s credential.
